REGISTRATION WILL OPEN DECEMBER 8TH.
Whit Church will be performing basic cardiac screening at the clinic.
NO REFUNDS FOR CANCELLATIONS, MISSED OR LATE APPOINTMENTS.
All health testing other than titers and heartworm tests are only available to AKC registered dogs, or dogs registered in an AKC accepted foreign registry. Please provide your dog's registration number on the dog information form.
The purpose of the health clinic is so breeders of purebred dogs can screen their breeding stock. Thus, appointments are long enough for the vet to do the test, and provide the results only. Should the test reveal an abnormal result, for further information and/or more screening please schedule with the vet during their normal business hours.
Please make sure to complete the dog information form AFTER YOU HAVE PAID FOR YOUR TESTS. We must have a valid e-mail address and telephone number, this is how you will receive your appointment times. Also if one person is paying for services but another person is bringing the dog(s) please cross reference names on everything.
General information...
We will only accept paypal, visa, or master cards . Visa and MasterCards are processed via the paypal button at checkout.
You will be notified of your appointment time THE WEEK PRIOR to the clinic.
All testing by appointment only. All appointments will be made directly with the Cavalier King Charles Spaniel Club of Central Arizona, Inc. No appointments will be held until payment is received AND DOG INFORMATION FORM IS COMPLETED AND SUBMITTED.

Please arrive 15 minutes early and make sure to bring your completed OFA forms with you to the appointment. You will not be checked in until all forms are complete.
Please be advised, due to the nature of the health clinics, we are unable to provide the special documentation required by pet insurance companies.
Sedation is extra (required for dogs over 80 lbs or at doctor’s discretion), priced on weight and will be payable directly to the vet at the time of service.

For all OFA tests please click on the button to the left to be directed to the forms page on the OFA website. You can either fill them in on line and print it off, or you can print it off and then complete it manually. DO NOT CHOOSE THE OPTION "SUBMIT ON LINE" ON THE OFA WEBSITE. Make sure to bring all completed forms with you to the health clinic.
For titers and heartworm tests please click on the button to the left, print it off, complete it, and bring it with you to the clinic. Please note, due to the nature of a health clinic, you and your dog(s) do not become a client of Applewood. Legally Applewood can't interpret the results...if you need them interpreted you will have to obtain it from your regular veterinarian.
X-Ray prices are for the radiograph/exam ONLY. If you plan to submit your radiographs to OFA, you will pay OFA directly by including your credit card number at the bottom of your OFA form(s) NO CHECKS OR CASH WILL BE ACCEPTED. Radiographs will be electronically submitted to OFA by Applewood and the corresponding forms will be faxed to OFA at the same time.

RABIES TITER: The Rabies titer we offer can NOT be used for the purpose of exporting a dog outside the U.S.
Your purchase is not considered complete until we receive a completed dog information form
We only take PayPal, Visa or Mastercard
The charge for your payment will appear on your PayPal or credit card statement as "Cavalier Club AZ" and will not directly refer to the Health Clinic. If you do not understand the charge, please contact us at cavalierclubofarizona@gmail.com for clarification before filing a dispute. If a dispute is filed through PayPal, you will be responsible for the additional fees incurred and we further have the right to cancel your appointments in their entirety.
